Transitions for Youth is a treatment foster care network.  
We have been licensed in the state of Ohio since 2002.  Our
agency is IV-E reimbursable and is a non-profit
organization.  We are very active and keep our agency small
so we can accomodate the needs of the children we serve.  
Children are given individual attention during home visits
and activities with their case manager.  They also
participate in group support twice a month.
Our Goal:
To provide the best possible
services and support to the
children, foster parents,
biological parents and custodial
agency.
Transitions For Youth accepts
children birth through eighteen
for placement into traditional
foster homes, treatment foster
homes or medically fragile foster
homes.  We strive to maintain
stability and work on building
positive relationships between all
members of the treatment team.
Case managers do not have set
hours.  They work into the
evening to serve the needs of the
children.
